11. Adoption of Retirement Contribution Rates Recommended by the Ventura County Employees' Retirement Board as Set Forth in the Ventura County Employees' Retirement Association Annual Actuarial Valuation and Review for Year Ended June 30, 2018.
DISTRICT ATTORNEY
12. Approval of, and Authorization for the District Attorney’s Office to Accept, a $10,000 Stipend from Camp HOPE America to Defray the Costs of Camp for 2019 Campers; and Approval of, and Authorization for the District Attorney to Execute, the Camp HOPE America Affiliate Participation Agreement and Readiness Grantee Memorandum of Understanding.

Approval of, and Authorization for the Health Care Agency (HCA) Director to Sign, on Behalf of the Ventura County Health Care Plan (VCHCP), Amendment Three to the Health Services Agreement Adding a New Fee Schedule with Children’s Hospital Los Angeles, Effective April 1, 2019; Approval of, and Authorization for the HCA Director to Sign, on Behalf of VCHCP, the Second Amendment to the Hospital Services Agreement Adding New Schedule of Reimbursement and Locations Exhibits with Simi Valley Hospital & Health Care Services, Extending the Agreement, Effective April 1, 2019 through December 31, 2019; Ratification of Continued Performance Under the Agreement with Simi Valley from January 1, 2019 through March 19, 2019; Approval of, and Authorization for the HCA Director to Sign, on Behalf of VCHCP, the Ancillary Care Services Agreement with Aspen Surgery Venture LLC Doing Business As Aspen Surgery Center, Effective April 1, 2019; Ratification of the HCA Director’s Execution, on Behalf of VCHCP, of Amendment Number Seven to the Pharmacy Benefit Management Agreement for the Provision of Certain Prescription Drug Benefit Management Services, Adding the Advanced Opioid Management Program, with Express Scripts, Inc., Effective April 30, 2019.

17. Approval of, and Authorization for the Health Care Agency (HCA) Director to Sign, 13th Amendment to the Provider Agreement to Add Primary Care Physicians with Valley Care Physicians IPA Medical Associates, Inc. Doing Business as Valley Care IPA, Effective June 1, 2019; and Authorization for the HCA Director to Make Changes to the List of Primary Care Physicians.

18. Approval of a Consulting Services Contract for the Bridge Road Bridge Rehabilitation Project, County Bridge No. 422, with Michael Baker International, Inc., in a Total Amount Not to Exceed $133,034; and Authorization for Public Works Agency Director to Execute the Contract; Contract No. AE19-029, Project No. 50562, Supervisorial District No. 3.
19. Authorization for the Public Works Agency Transportation Department Director to Accept Federal Highway Safety Improvement Program Cycle 9 Funds for Three Projects for Fiscal Year 2018-19 in the Amount of $1,323,000; All Supervisorial Districts.
PUBLIC WORKS AGENCY - Watershed Protection District
20. Approval of the Acceptance of $1,577,451 in Flood Emergency Response Projects Statewide Grant Round 3 Funding from the California Department of Water Resources on Behalf of Seven Agencies in Southern California, for Phase 3 of the ALERT2 Capacity Upgrade Project; Waiver of Policy Prohibiting the Inclusion of Attorney's Fees Provisions in Contracts; and Approval of, and Authorization for the Ventura County Watershed Protection District Director to Execute the Grant Agreement; All Watershed Protection District Zones; All Supervisorial Districts.
